Atopic dermatitis, the more you worry the more it comes
In autumn, many patients complain of itching. Atopic dermatitis among children is getting more serious. With more than 1 million patients suffering from atopic dermatitis nationwide, we came and talked to Dr. Pyeon Bok-yang, at Soonchunhyang University Seoul Hospital.
Q. Park Bo-hyun, CEO of B&H Korea (hereinafter referred to as Park Bo-hyun): Many patients complain of dryness and itching of their skin in autumn. How is itchy skin different from atopy?
A. Professor Pyeon Bok-yang of Soonchunhyang University Seoul Hospital (hereinafter referred to as Pyeon Bok-yang): Skin itchiness can be caused by various reasons. Typical causes include diseases such as atopic dermatitis, urticaria, and contact dermatitis, and medical conditions such as liver, biliary tract disease, chronic kidney disease, diabetes, and insect wounds and psoriasis can also cause itchy skin. The term "atopy" refers to an allergic reaction in which the causative antigen is identified. Therefore, atopic dermatitis uses the term atopic dermatitis. Atopic dermatitis is an allergic skin disease accompanied by severe itching, and allergic reactions are secreted into the body due to allergic reactions, which accompany itching. It is characterized by severe itching, eczema, and dry skin.
Q. Park Bo-hyun: Atopic dermatitis and cure, can these words be used together?
A. Pyeon Bok-yang: Atopic dermatitis is not a simple skin disease, but an allergy, so it is not something to be cured but a controllable infant disease. About 50-60% of atopic dermatitis begins in infancy, and as they grow, it improves over time, and many of them outgrow it when reaching adolescence. However, children's allergic diseases are characterized as "atopic march", which means even if atopic dermatitis improves, they tend to have an allergy such as asthma and allergic rhinitis.
Q. Park Bo-hyun: 45% of atopic dermatitis patients are children under the age of 10. What is the biggest cause of atopic dermatitis in children?
A. Pyeon Bok-yang: Atopic dermatitis is the most common form of allergic disease along with asthma and allergic rhinitis. In particular, as it appears in infancy and among toddlers, it can be recognized as the starting point of allergy. About 60% of children with atopic dermatitis grow and develop respiratory allergies such as asthma and allergic rhinitis. In other words, compared to other allergies, atopic dermatitis often occurs at an early age, which varies depending on the cause and type of worsening antigen in people with allergic tendencies, and causes other diseases on the skin, respiratory tract, and conjunctiva.
Q. Park Bo-hyun: Is there a way to prevent pediatric atopy that usually appears two to three months after birth?
A. Pyeon Bok-yang: Atopic dermatitis is a disease that occurs in people with allergic tendencies, so, unfortunately, there is no cure. However, since environmental factors such as diet, living conditions, and air pollution are involved in the recent increase in allergic diseases including atopic dermatitis, managing these factors would be an effective preventive measure.
Research shows that high-risk infants and children with allergic diseases such as atopic dermatitis, asthma, and rhinitis are recommended to be breastfed up to at least 4 months old if possible, and that moisturizing the skin also helps prevent atopic dermatitis. In addition, it is also helpful to prevent dust mites and fungi in possible.
Q. Park Bo-hyun: The number of adult atopic patients in their 20s and 30s continues to increase. I heard adult atopy is more dangerous than infant atopy, why is that?
A. Pyeon Bok-yang: Adult atopy is often not well controlled. About 50% of pediatric atopic dermatitis patients naturally improve by 6 to 7 years old or when reaching adolescence, but atopic dermatitis in adults would not improve by itself. In addition, stress and psychological burden are also factors that worsen atopic dermatitis, making it more difficult to control in adult patients. Patients with atopic dermatitis also receive medication to control itching because severe itching causes sleep disorders. However, antihistamines that can control itching have a side effect of severe drowsiness in adults, causing inconvenience in their academic and social lives. As such, adult atopic dermatitis patients are often more difficult to control due to difficulties in treatment due to various causes and exacerbation factors, and restrictions such as drug selection.
Q. Park Bo-hyun: It sounds like that steady management of atopic dermatitis is most important. What are some management methods that patients can do at home?
A. Pyeon Bok-yang: In order to recover healthy skin, reducing irritation by relieving the itchiness caused by Atopic dermatitis is the key. Therefore, skin moisturization and cleanliness are important as a way to control itching. As patients with atopic dermatitis have a damaged skin barrier, the moisture in their skin evaporates a lot and the ability to retain has decreased a lot. Also, there are better chance of infection or contamination through damaged skin. Therefore, it is important to moisturize your skin frequently with the appropriate moisturizer. The choice of moisturizers may vary depending on the patient's skin condition, season, indoor and outdoor environment, occupation, and preferences, so consulting with an expert may be helpful.
Q. Park Bo-hyun: What would you like to say to the 1 million patients suffering from atopic dermatitis and their caretakers who also suffering together?
A. Pyeon Bok-yang: Atopic dermatitis is a disease that can be effectively controlled. If you treat it consistently based on the principle, you can maintain your normal skin condition and lifestyle. Therefore, it is important to treat in principle rather than indiscriminate treatment with inaccurate information or untested methods. In particular, parents with children who are suffering from atopic dermatitis should be careful because they often limit their diet indiscriminately based on unconfirmed information, leading to malnutrition disorders such as anemia and growth retardation.
